After numerous signing in and out and restarts and deactivating and reactivating the iCloud Add-In through Outlook, I finally uninstalled & reinstalled iCloud for Windows and all was good again.

My wife's ancient Sony Vaio running a free upgrade W10 Pro and the same Office 365 subscription didn't offer me the link to "Set up iCloud Contacts, Calendars and Tasks in Outlook" in iCloud settings window.

I had this issue after W10 Anniversary update (1607) on 2 PCs in our household, but with an error message "The set of folders cannot be opened. iCloud for Windows 10 repair and installation failures are fixed by uninstalling Outlook and all Apple related software completely and then re-install them.
